The rise of AI agents in healthcare: Designing man-machine systems.

Most people think AI in healthcare is about replacing doctors - that will be too narrow and missing the point.

It’s not.

It’s about redesigning how decisions get made.

I took the approach that AI in healthcare is not about replacing doctors but augmenting the clinicians or the eco-system. I then layered over theory with a practical approach to implementation - The Clinical Agent Stack. It is about designing systems where humans and machines work together.

In my e27 piece, I introduce the idea of a Clinical Agent Stack:
→ Execution Layer: AI handles tasks
→ Optimization Layer: AI allocates resources
→ Decision Layer: Humans retain oversight
→ Learning Layer: Systems improve over time

the clinical agent stack

AI will not replace clinicians.

And clinicians who work within well-designed AI systems will redefine the standard of care.

The real shift isn’t automation.

It’s orchestration.

The hospitals that win won’t be the ones with the best AI models.

They’ll be the ones that design the best man–machine systems.

That’s the real operating model shift.
